MSPSL and US Club Soccer registration will begin immediately after teams have been chosen.  League Registration forms must be signed immediately after try-outs and initial payment must be received before positions can be held.

LEAGUE PAPERWORK

Our club participates in three different leagues.  Check which league(s) your team is playing in and fill out the proper paperwork.  Ifyou are unsure of the league you are playing in please check with your coach or refer to the try-out information page.

Champions League Registration (USACL) Player Registration:

#1 US Club Soccer Registration Form

#2 Copy of Birth Certificate

#3 One Small Pictures (2"x2")

#4 Signed Player/Parent Contract (.pdf)

#5 Check payable to Alliance FC for player fee.  (If you are choosing the payment plan parents must sign a payment plan contract).   

Grand Valley Soccer Association (GVSA) Player Registration:

#1 GVSA Registration Form

#2 MSYSA Medical Release Form (MUST BE NOTARIZED)

#3 Copy of Birth Certificate

#4 One Small Picture (2"x2")

#5 Signed  Player/Parent Contract (.pdf)

#6 Check payable to Alliance FC for player fee.  (If you are choosing the payment plan parents must sign a payment plan contract).

Michigan State Premier Soccer League (MSPSL) Player Registration:

#1 MSPSL Form (only available at registration parent and player must sign).

#2 MSYSA Medical Release Form (MUST BE NOTARIZED)

#4 Two (2) Small Pictures (2"x2")

#5 US Club Soccer Registration Form

#6 Signed Player/Parent Contract (.pdf)

Coach Registration:

#1 GVSA and MSPSL coaches must have a risk management card.

#2 MCL and US Club Soccer requires a coaches Risk Assessment Form.
