Permission to Travel Forms are needed for Alliance FC teams that are registered only under MSYSA and are traveling out of state for league or tournament play.

Teams that are carded through US Club Soccer do not need travel forms and can obtain additional information by visiting www.usclubsoccer.com.

Managers of teams playing only in GVSA or MSPSL will need the following to play in an out of state tournament or league game.

1. Click on Permission to Travel to US Youth Soccer Tournaments

This form can be filled out online. There is a $50 charge per team.

2. Consent to Travel forms must also be completed for each player.
